After October 27th, your health records from my practice will be transferred to Morgan Records Management, LLC, who will serve as the records custodian. Morgan Records will maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed either seven years from the date of your last encounter with my practice or until your child reaches the age of eighteen, whichever is the longer retention period.
